Sun Health At Home - services

Sun Health At Home® offers personalized support and care, backed by a commitment to quality that has made Sun Health a trusted name in wellness and senior living services.

As your health needs change, a Wellness Coordinator will act as your partner and personal advocate. They ensure members are prepared, aware of their options and always able to access the care and services they need. In addition, Sun Health At Home also provides non-medical services such as transportation, homemaker and companion services, and referrals to vetted service providers so members and their families don’t have to coordinate services, handle phone calls or paperwork. Someone from the Sun Health team is always available to assist you 24 hours a day, seven days a week.

Health And Wellbeing Services

Members have access to wellness services, consultative services, healthy living classes and educational resources that tackle some of the most common health conditions that affect the local community.

In-Home Functional And Safety Assessment

Members can receive an annual evaluation to assess functioning and safety within their home. We may recommend supportive technology that enables members to live safely and independently, such as devices that provide medication reminders or assist in dispensing medication.

Access To Sun Health Community Locations

Members have guest privileges at any Sun Health Community location—Grandview Terrace, La Loma Village and The Colonnade—to take advantage of dining, fitness and social opportunities. Should a member require a higher level of care than is available in their home, their Wellness Coordinator will arrange and provide support to ensure the transition from their home to the level of care that best meets their needs is seamless.

Emergency Response System

An emergency response system is provided to give members and their families peace of mind. Using GPS technology, members can speak directly to a response agent for assistance with the touch of a button.

In-Home Assistance

When medically necessary, members receive assistance while at home including medication management, meal preparation, transportation and more. Assistance can be provided through:

  • Companion services
  • Homemaker services
  • Personal caregiver services

Referrals And Benefits Coordination

A Wellness Coordinator helps members understand their options and coordinate benefits by utilizing the plan prepared in advance to assist members in navigating the healthcare system, understand their insurance coverage and identify needed services to facilitate their health and wellbeing. They also recommend and identify home service providers for services such as:

  • Home and landscape maintenance
  • Legal assistance
  • Financial planning
  • Medical equipment rental
