Life Plan Communities, formerly known as Continuing Care Retirement Communities, offer independent living along with all levels of health services under the umbrella of one community. Sun Health Life Plan Communities includes all three Sun Health locations as well as our Sun Health At Home® program.

Independent Living
Independent living offers residents the convenience and enjoyment of living in their own private residence at one of our three Sun Health Community locations—all while knowing they have access to a wide range of health services if or when they need it.
Sun Heath At Home assists in keeping members well, active and healthy while living independently in their own residence with coordinated services and the support of a personal wellness coordinator so that they can successfully age in place.
